The services that a PCA provides may enable the individual to remain at home rather than in a group home or other environment.

Our program carefully matches each client with a trained PCA. Types of assistance provided by a PCA will vary based on individual need, but may include:

  • Daily living and health-related tasks
  • Supervision for health and safety
  • Behavior redirection/intervention
  • Assistance with participating in home and community settings

To qualify for our PCA services, an individual must:

  • Be eligible for Medical Assistance (MA) or TEFRA
  • Receive an assessment from a Public Health Nurse
  • Live in Hennepin County
  • Choose St. David’s Center as the service provider

Payment/Funding

We accept Medical Assistance (MA), TEFRA, HealthPartners, MEDICA, and private pay.
